Epilepsy.com/professionals is designed so that you are not required to provide any personally identifiable information unless you are requesting access to certain features and services or you are voluntarily submitting personally identifiable information to us. Personally identifiable information may include a User's name, address, telephone number, credit card number, e-mail address and any other personal information that could be used to contact or identify you that you provide through the Site to us, our employees or agents, via e-mail or otherwise. This also includes personal information that you provide to us by mail, such as a donation. When this information is combined with any information about your health or medical condition, we refer to the resulting information as health-related personal information.
The Site may allow you to register yourself to access certain pages or services. For example, the My Epilepsy Diary feature enables patients to register at our web site where they can create an individualized profile, which may include information relating to their experience with epilepsy including seizure episodes, medications and side effects.

How We Use the Information
Personally Identifiable Information. The Epilepsy Therapy Project uses the personally identifiable information collected from its Users to contact you confirming your registration with the Site, respond to your questions and/or comments, process a donation, fulfill a purchase request and notify you of your order status, provide any services or information that you request through the Site. Upon your request, we may also provide you with information about epilepsy that we believe would interest you, such as information from the Epilepsy Therapy Project, or provide you with information about important functionality changes to the Site.
Health-Related Personal Information. If you choose to participate in our interactive services, we will collect the information you disclose to us and analyze it to discover new facts that could help us better understand how our services are used in coping with epilepsy. We may offer interactive services such as the My Epilepsy Diary that involve health-related information. We may combine, in a non-identifiable format, your health-related information that you provide with information from other Site users to create aggregate or summary data that we will use for our organization's purposes and to support our mission, including research to improve epilepsy care and analyses that may help us offer better support services.
Who We Share Your Information With
We will not sell, share, trade or give away to third parties outside Epilepsy Therapy Project any personally identifiable information that we collect or receive regarding Users of Epilepsy.com/professionals without the Users' permission except as explained below:
Applicable Law/International Issues
Information that is submitted to this Site will be collected, processed, stored, disclosed and disposed of in accordance with applicable U.S. law. If you are a non-U.S. User, you acknowledge and agree that we may collect and use your Information and disclose it to other entities outside your resident jurisdiction, as discussed in "Who We Share Your Information With" above. In addition, such Information may be stored on servers located outside your resident jurisdiction. U.S. law may not provide the degree of protection for personal information that is available in other countries. By providing us with your Information, you acknowledge that your have read this Policy, understand it, agree to its terms and consent to the transfer of such information outside your resident jurisdiction. Children's Privacy Protection
Epilepsy.com/professionals is a general audience web site not geared towards children. We are committed to protecting children's online privacy and this Site does not knowingly collect any personal information from children under the age of 13. If you are under 13, you agree that you will not provide personal information about yourself to The Epilepsy Therapy Project through the Site. Please contact us at the email provided in the "How to Contact Us" section of this Policy if you believe we may have collected information from a child and we will work to delete it.
Your Privacy Choices
We would like to send you information about our organization and our services if you wish to receive such information. When you register on our Site, you will generally be given the choice to "opt-in" to receiving such information from our Site, but if opt-in choice is not provided, you may opt-out of promotional communications at any time. Epilepsy Therapy Project makes no representations or warranties with regard to the sufficiency of these security measures. Epilepsy Therapy Project shall not be responsible for any actual or consequential damages that result from a lapse in compliance with this Policy as a result of a security breach or technical malfunction. Internet transmission, including e-mail transmission, is not secure and as a result, we cannot guarantee the security of our database, web site or services, nor can we guarantee that the information you supply or that we send to you or on your behalf will not be intercepted while being transmitted to us over the Internet.
